Biographical Sketch: Theresa Countryman graduated from Townson State University with a BSN in Nursing in 1988. She began her nursing career at University of Maryland Medical System as a medical-surgical nurse on a unit with a focus on infectious disease including HIV/Aids patients. After several years as a stay at home mom, Theresa worked at a long term skilled nursing facility for 3 years. In 2006, Theresa came to Gilchrist Hospice Care as a Case Manager for Home Care and was certified in hospice and palliative care in 2007. In April 2010 Theresa became the Home Care Clinical Lead. In this position Theresa is responsible for precepting new Case Managers and providing clinical support to home care nurses. In addition to this role, Theresa also participates in Pharmacy and Therapeutics committee and in continuing quality improvement efforts.
