Biographical Sketch: Dr. Mittelberger is an Internist, Geriatrician and Hospice and Palliative Care physician. He currently serves as National CMO for Evercare Hospice & Palliative Care. He is Chair of the California State POLST Task force and has been a physician leading POLST implementation in California since 2007. Prior roles include West Region CMO for UnitedHealthcare's Medicare Division and Chief of the Division of Geriatrics and Palliative Care at the Alameda County Medical Center for 20 years where Dr. Mittelberger was responsible for resident education, ethics and clinical consultation, and had an active clinical practice from hospital to SNF to home care. He is a Clinical Professor of Medicine at the University of California, San Francisco. Academic interests include advance care planning, clinical ethics, Alzheimer's disease, quality improvement, and integration of palliative care, geriatrics and hospice into systems of care. He is also President-elect of California Association of Long Term Care Medicine. (State Chapter of American Medical Directors Association.) He has been actively engaged in discussion and teaching about POLST to numerous medical staffs, nursing and medical organizations, state and federal regulatory bodies, and others.
