1. Describe the impact of falling on hospice patients and families
2. Identify risk factors for falling for hospice patients
3. Discuss interventions to prevent falls and/or decrease injury from falls for hospice patients
There is little research regarding identification of fall risk in the hospice population, nor what interventions are successful in preventing falls and/or decreasing injuries from falls in this population. This presentation will discuss the process of identifying an appropriate fall assessment tool and designing a process to utilize evidence-based interventions which are individualized to a specific patient’s risk factors to help identify and prevent falls and/or injury from falls.
