1. Describe the similarities and differences between the PACE and hospice models of care
2. Identify two key pitfalls to avoid when building collaboration between a hospice sponsor and PACE
3. Recognize the regulatory mandates and practical considerations regarding end-of-life care that affect PACE
Drawing from years of experience in the Program of All-inclusive Care of the Elderly (PACE) and also in a hospice-sponsored PACE organization, the presenters will briefly describe the PACE model and review the regulatory and practical aspects of palliative and end-of-life care in the PACE setting. As more hospice organizations develop or sponsor a PACE, practical issues around collaboration and competition become relevant. This session will explore how key ideals were envisioned in one hospice-sponsored PACE and how the dynamics played out in reality.
