1. Identify the barriers to effective team meetings and methods for overcoming these barriers
2. Describe how team meetings may be more effectively and efficiently managed by utilizing a defined agenda and patient presentation templates
3. Define the roles of the team meeting facilitator(s) and explore options for choosing appropriate facilitators
Unfortunately due to lack of adequate knowledge and skills, IDT meetings are often characterized by monopolizing medical/nursing issues, lengthy eligibility debates, tangential discussions and a lack of interdisciplinary participation.  This interactive presentation will summarize how one hospice used an interdisciplinary approach in identifying the IDT meeting problems and developing a process improvement plan to reclaim team meetings for their original intent, to update the plan of care based upon what’s most important to the patient and family. Faculty will share training materials, templates for the team meeting agenda and patient presentations, and a responsibility list for the entire interdisciplinary team entitled “Pathway to a Successful Team Meeting-8 Key Steps”.
