This session will demonstrate how to function well as an interdisciplinary team where the focus is on the patient and the caregivers.  This is a MODEL of clinical excellence to assure we function well as TEAMS.  This MODEL is used for the management of physical, emotional and spiritual distress to alleviate suffering.  The TEAMS assure that the expertise of all disciplines is utilized for the timely and thorough coordination of care.
