Biographical Sketch:
Lori Ann Attivissimo, MD, FACP, FAAHPM Lori Ann Attivissimo is Senior Medical Director with Hospice Care Network and the Miracle Foundation Home Based Palliative Care Center in Woodbury, New York. She is also the Medical Director for Post-Acute Services for the North Shore-LIJ Health System. Dr. Attivissimo graduated from Wayne State School of Medicine in 1990. She completed an Internal Medicine Residency at North Shore Manhasset in 1993, followed by a fellowship in Medical Oncology and Hematology in 1996. She was an attending in Medical Oncology/Hematology at North Shore University of Glen Cove until transitioning to Hospice Care Network as a Hospice Medical Director in 2000. She is a fellow of the American College of Physicians (FACP) and American Academy of Hospice and Palliative Medicine (FAAHPM), board certified in Internal Medicine, Medical Oncology and Hematology, Hospice and Palliative Medicine, and Hospice Medical Director. As an assistant Professor at the Hofstra North Shore-LIJ School of Medical she teaches patient/physician communications in their Challenges, Privileges and Responsibilities (CPR) course. She was trained in the Education of Palliative and End-of-Life Care (EPEC) project. She is an active participant and presenter in the Hospice and Palliative Care arena. She is on the Hospice & Palliative Care Association of NY State (HPCANYS) State Legislative/Regulatory Subcommittee. Dr. Attivissimo is committed to the further growth, awareness and development of Conversation Ready Education and End of Life Care.
