Biographical Sketch:
Jennifer has been with Hospice of the Western Reserve for 14 years. Originally, in the role of a Professional Relations Coordinator, Jennifer acted as a liaison for Hospice of the Western Reserve for area hospitals, nursing facilities, physicians and the healthcare community that she served. Jennifer also provided educational in-servicing regarding end of life care. With over 10 years as a paralegal in the elder law arena and her tenure at Hospice of the Western Reserve, Jennifer has spoken to many diverse audiences as she reached out to provide information about access to palliative and hospice care. Jennifer has presented to the National Hospice and Palliative Care Work Group in Orlando Florida in 2003, in Chicago Illinois in 2004 and a national audio-conference regarding collaborative community partnerships for the National Hospice and Palliative Care Organization in 2005. Jennifer has also presented at the AOPHA conference in 2004 and 2005 and presented to the National Hospice Work Group in Nashville, Tennessee in May 2006 and the National Hospice and Palliative Care Management Conference in Washington D.C. in 2010. Jennifer became the Director of Referrals and Admissions in 2007, overseeing access to care and the admission teams for the agency with an average daily census of over 1,150 hospice patients. In 2013, Jennifer accepted the position of Director of Access to Care, overseeing the admission teams and nursing facility teams. Jennifer received her paralegal certification from Lakeland Community College, a Bachelor of Arts degree in Urban Studies from Cleveland State University in Cleveland Ohio and a Master of Science in Health Science degree in End of Life Care Management from the George Washington University in Washington D.C.
