The healthcare delivery environment is changing and care providers are being asked to do more with less, to spend more time in managing care versus providing care and to focus on the goals of patients and their caregivers in a broader perspective. Hospice is a care provider model wrapped in a care manager framework, which positions hospice and palliative care organizations perfectly to respond to the new norm of patient-centered care in the broadest sense. This session explores how hospices can be excellent healthcare partners while living up to the mission of supporting holistic care.

2. Discuss trends in healthcare delivery which impact hospice and palliative care
