GIP Stakeholders: Who Cares and Why It Matters

This session will examine the provision of the hospice inpatient level of care with identification of stakeholders and exploration of strategies to address their perspectives. Additionally, faculty will review the challenges in transitioning patients to the appropriate level of hospice care while describing strategies that managers can use to strengthen the collaboration and documentation that supports the provision of general inpatient level of care and discharge planning for transition to a lower level of care when appropriate.
