From Fringe To Core, A Story of Radical Change in Patient Care

Managing the needs of nearly 2,000 patients per day and 9 inpatient units required a new approach to patient care. Learn one organization's story on how they kept pace with rapid change and the need for consistent high-quality service in today’s hospice environment. A new team was created that would manage every patient’s needs without delay. Simplifying the patient experience to a single phone number, with 24-hour support for every need, the patients discovered they could have an emergent needs nurse in 90 minutes, medication refills and equipment within an hour, and calls when they (or their caregivers) just needed advice and support were answered immediately with a qualified and compassionate “team” nurse.
