The Centers for Medicare and Medicaid Services (CMS) has stated that 90% of all payments will be tied to fee-for-service by the end of 2018 with a link of payment to quality or population-based payment. Whether or not CMS completely restructures the way hospices are reimbursed, the shift to value-based care will change the practice and referral patterns of other providers. Value-based care is the impetus for the development of post-acute care networks so that health systems can control costs and improve continuity of care across the care continuum. This session will explore ways to strategically lead your hospice through this transition period by strengthening your marketing position, focusing on the metrics important to payers and providers, preparing staff and board members for this evolution, and developing business plans that are easily adaptable depending on internal and external drivers of change.
