Community based palliative care presents many challenges. Chief among these challenges are financial viability and maintaining operational efficiencies within an interdisciplinary framework. The SPARK Program met these challenges while addressing the need for collaboration across care settings and across care providers. This presentation will briefly describe the development and ongoing changes to the model as well as a case study to demonstrate the program in action.
