In 2008, HPC Healthcare, Inc. was hit by the perfect storm. Increased scrutiny of documentation by Medicare Fiscal Intermediaries and changes in the Medicare Conditions of Participation led us to evaluate our documentation practices. Based on this evaluation, we noted a need to change the way we document in order to clearly demonstrate patient hospice appropriateness, provide care to address patient needs and to support reimbursement. To meet this goal, a multi-faceted program including staff education as well as changes in forms was implemented. In this  session, we will describe the development, implementation and evaluation of our documentation education system that demonstrates staff’s ability to document to standards.
