1. Identify benefits of consistency and coordination of care in children with complex medical conditions, including those at the end of life
2. Identify barriers to optimal transitions in care
3. Develop strategies to ensure seamless transitions of care
The goal of this presentation is to facilitate consistent, comprehensive palliative care no matter where a child is in the trajectory of illness, by whom he/she is being cared or where care is provided. By focusing on transitions over time, between care providers and between sites of care, quality and satisfaction with care will be optimized.
