NorthShore Hospice’s "Transitions Packet" fills an often overlooked gap in the continuum of care. The packet, provided at the time of admission by the social worker, represents an expansion of bereavement support services both pre and post loss. Containing checklists, worksheets and prose, this resource provides structure for families as they negotiate the maze of financial, legal, social, emotional and spiritual concerns that accompany the death a loved one. The "Transitions Packet" provides interdisciplinary team members with addtional places for meaningful intervention.
