1. Identify the hospice discharge regulations and process for live discharges
2. Identify when to issue the Advance Beneficiary Notice for hospice discharges
3. View the hospice discharge from the patient's shoes and initiate appropriate measures for discharge planning
Live discharges from hospice care are becoming a more frequent occurrence due to greater emphasis and measures to ensure that a patient meets eligibility criteria.  This session will focus on the hospice discharge process including discharge planning, issuance of the ABN and patient counseling about discharge for hospice care.
