Many patients are admitted into hospice services already taking long-term anticoagulation therapy to prevent or treat thromboembolism.  A thorough understanding of the available anticoagulant therapies is crucial for hospice health care professionals to safely manage their patients.  This evidence-based, case-based presentation will discuss the available anticoagulant therapies, dosing strategies, monitoring and patient education.  Faculty will also evaluate the reasons for initiation, continuation or discontinuation of anticoagulation therapy in the hospice setting.
