1. Recognize common deficits in documentation of spiritual care in the hospice setting
2. Restate patient/client problems and outcomes in behavioral language that is discipline-specific
3. Describe documentation tools that support ongoing assessment, individualized care and documentation which reflects the impacts and quality of spiritual care in the hospice environment
Documentation of spiritual care too often fails to effectively reflect the quality of care provided by spiritual caregivers at end of life. This presentation will explore strategies that have been developed and implemented at The Hospice of Dayton that have markedly improved case documentation of spiritual care while protecting patient privacy. Development of a structured documentation tool and as well as discipline-specific behavioral language to communicate care focus, interventions and outcomes will be discussed as an alternative to vague, generic, non-specific documentation practices.
