"Continuum of care" describes a community of healthcare providers that work together to provide seamless, coordinated care to address the comprehensive needs of patients/families.  In some communities one or two agencies offer services along the continuum while in others, multiple organizations collaborate.  In either model, how patients are transitioned to palliative care is explored throughout the disease trajectory. This session will help you identify innovative approaches that expand hospice and palliative care services.  NHPCO’s Continuum of Care Assessment Tool and Guide will be highlighted and specific strategies and resources to help meet the unique needs of patients and families in your community will be highlighed.
