As hospices strive to find innovative ways to provide palliative care, creative partnerships that serve all involved parties are essential.  Southern Tier Hospice and Palliative Care has developed a rich and powerful partnership with a local health care system to provide inpatient palliative care, including a hospice social worker and nurse who work daily with the hospital staff to establish relationships with the patient and family in the hospital setting.  Patients, families, the hospital, physicians and the hospice have seen great benefits. Faculty will provide background and details about the business planning that allowed this to happen.
