Learning Objectives:
1. Describe the end stage disease process and clinical course for common conditions such as dementia, debility, COPD, and heart failure and associated common co-morbid and secondary conditions
2. Name the clinical data points necessary to substantiate hospice eligibility for dementia, debility, COPD and heart failure, including the appropriate/required assessment scales
3. List best practices for documentation of eligibility and recertification and provide sample narrative notes that support admission and recertification
Paricipants of this workshop will learn the criteria required for hospice admission and recertification of patients with dementia, debility, COPD and heart failure based upon Local Coverage Determination (“LCD”) guidelines. Case examples will be used to illustrate best practices for assessing and effectively documenting clinical indicators necessary to substantiate hospice eligibility for common disease states. Documenting clinical eligibility is one of the most targeted areas for hospice audits.
