Improving Late Stage Disease Management and End-of-Life Care by Understanding the Emerging Diagnosis of Geriatric Frailty Syndrome

1. Discuss the ability to define the frailty phenotype
2. Understand the pathophysiology of frailty
3. Describe the prognostic significance of frailty
Hospice professionals need to understand the medical evidence supporting Geriatric Frailty Syndrome and they should be able to translate that understanding into better late-stage disease management, palliative and hospice care. In addition, hospices should be able to provide treatment options for patient and family consideration that are evidence-based and appropriate for this time in the life-cycle. Faculty will describe how Sharp HospiceCare has applied this diagnosis to its state-of-the-art outpatient palliative care and hospice programs to improve patient outcomes, increase appropriate utilization of services and improve patient and family quality-of-life.
