Management’s role is to support quality care and outcomes at the same time ensuring documentation supports eligibility. All too often clinicians separate eligibility from care planning and care planning from patient outcomes. Having a dynamic plan of care helps support eligibility and is used as a road map for all members of the interdisciplinary team.  This session explores the fine art of tying eligibility assessment to effective care planning and provides strategies to help managers support staff in making the connection so all eligible patients receive the care they need.
