As polypharmacy is recognized as an increasingly frequent problem in patient care, the goal should be not to over-prescribe, nor under-prescribe, but to judge which are the appropriate medications for patients in the context of their current illness. This session will review the evidence on appropriate prescribing of pharmaceuticals with a focus on selecting the right medications for patients with advanced illness based on the current literature. In addition, a tool to review pharmaceutical provision will be presented as well as practice cases.

Learning Objectives:
1. Describe the evidence underlying the problem of polypharmacy and be able to review the medication profile of patients
2. Apply a simple tool for the review and management of pharmaceuticals
3. Discuss the rationale behind deprescribing to patients and families to obtain buy-in of the process
