Integrating palliative care into your community based home health and/or hospice program can be challenging. Competing business drivers often derail good intentions preventing patients from getting the right care at the right time. This session will examine the development and implementation of a progressive illness management program. The program uses an advance practice nurse led model that supports the medical management of patients throughout their disease trajectory and the skilled intermittent home health care needs of patients to ensure timely referrals to home health and hospice services. Faculty will share successes and lessons learned as they relate to clinical services, operational and financial considerations.
