Post-acute-care will likely be the new paradigm that encompasses hospice, home health, skilled nursing, long term acute care (LTAC), and inpatient rehabilitation. Collaboration among these providers will be essential; the organizations that prevail in the new order will be those that have successfully “reached across the aisle” to engage, collaborate, and perhaps merge their operations. Participants will be shown strategies for upgrading their hospice programs to full-fledged Post-Acute-Care organizations.
