Effectively coordinating care is key to the success of the  high-level goals of reform, expand access, control costs and improve the delivery of care. Hospice and palliative care organizations are poised to make a significant impact on the delivery of high quality, cost effective and coordinated care in communities across the country. An important first step is to engage two important audiences – physicians and C-Suite level executives in hospitals and health systems.
