Discussing death and dying is typically an easy and comfortable discussion hospice staff are able to have with patients and families.  However, discussing the need to discharge a patient from hospice care can often be riddled with anxiety and confusion. A recent study by Abt Associates indicates that approximately 49% of live discharges from hospice care die within 6 months of discharge. If the discharge from hospice is not properly handled, many patients and families may go without hospice care at the time they need it the most. Having a consistent discharge process and staff who are as comfortable having the discharge discussion as they are the death and dying discussion can help ensure that patients and families are receiving hospice care when needed most.
