Few options exist for outpatient palliative care for patients requiring follow-up after receiving inpatient palliative care, or for patients who could benefit from palliative care services outside of the hospital.  This workshop will describe the development and start-up of an outpatient palliative care center that operates as a licensed community clinic, the first of its kind in California.  The Palliative Care Center Silicon Valley offers patients consultation related to advance directives and treatment options, as well as treatments to manage symptoms related to their serious illness. The Center offers an interdisciplinary approach to care which includes physicians, nurse practitioners, social workers, spiritual counselors, volunteers and case management services. Reimbursement for clinic services is available through Medicare Part B, Medi-Cal (Medicaid) and private insurance.
