Hospice and palliative care organizations are important components of the care continuum that is being developed by all risk-based payors, including bundled payment providers, ACOs, and health plans. This session will provide an update on the implementation strategies of accountable care payors across the nation. A key focus of accountable care payors has been the development of continuing care networks (CCNs) that begin with skilled nursing facilities (SNFs), home health, and increasingly include hospice and palliative care providers.
