National Harbor 11 (Gaylord National Resort and Convention Center)
Primary Presenter:
Mary Grace Mihalyo, BS, PharmD, CGP, BCPS
Area of Emphasis: Organizational Excellence
Learning Objectives:
1. Define hospice pharmaceutical care and MTM and align these with the expectations of CMS today
2. Identify 3 examples of explicit medication use criteria and understand the use of these tools in EOL care
3. Demonstrate a working knowledge of quality of life appropriate medication-therapy-management and the necessary patient/caregiver education demanded today
CMS continues to turn up the heat on the hospice industry with respect to total patient care and related medication use. It is no longer acceptable for a hospice organization to simply designate select medications as "patient pay" without requisite medication use follow-up and management. This session will explain and demonstrate total medication-therapy-management and appropriate hospice pharmaceutical care. Active learning techniques and case presentations representing complex symptom management issues will be employed to illustrate rapid response MTM required to competently meet the needs of the patient as well as the latest demands of CMS.
