1. Describe the difference between hospice and palliative care programs and describe models for PCPs,including Accountable Care Organizations and Medical Homes
2. Understand the legal issues that must be complied with when creating a PCP
3. Discuss the new Palliative Care initiatives in New York State to expand terminal prognosis to 12 months for Medicaid patients
Palliative care programs (PCP) (such as Accountable Care Organizations and Medical Homes are perfect partners for patient-centered and all inclusive care delivery models promoted by the Affordable Care Act.  Hospices and PCPs have the experience and knowledge to provide case management and coordinated care through interdisciplinary teams.  PCP are precursors to the ACO and Medical Homes and therefore, are in a position to partner with them.  What type of PCP is the best setting: physician group, home health agency, nursing home, contract arrangements, nurse liaisons, MCO, ACOs or Medical Home.  This presentation will describe the various ways to structure a PCP, the reimbursement revenue streams, the legal requirements, and new initiatives in New York state promoting palliative care.
