This session focuses on creating impactful and innovative partnerships and affiliations which broaden hospice's network, geography and continuum of care.  Participants will understand: the impact of health systems consolidation and network development; the impact of health system and payer initiatives on hospice and end-of-life care; the how and why of strategically positioning your hospice program; and evaluate best practice models.  Faculty will also discuss the challenges and successes of building out a continuum of care.
