"Check-in" and Improve Family Satisfaction of Hospice Care in Facilities

Many hospices utilize volunteers to "tuck-in" home patients prior to the weekend. This innovative program engages volunteers who "check-in" with primary caregivers of facility patients monthly. Volunteers use a rounding log and ask questions related to quality of care, improvement in pain and symptom control and communication. Caregiver concerns are identified and improvements are made immediately resulting in improved care and increased family satisfaction. In a quick 30 minutes you will go home with tools and procedures needed to implement this program in your hospice, at no cost.
