2015 brings a new opportunity for patient care and reimbursement: Chronic Care Management. This session will review this new Part B benefit along with the existing Transitional Care Management to help palliative care providers provide, document and bill these services correctly. With the growth of palliative care comes an increase in services provided by nurse practitioners or physician assistants and their collaborating physicians. When both a nonphysician practitioner and physician visit a patient on the same day, billing may be done under the physician’s provider number, but only if all the criteria for a split/shared visit have been met. The CERT contractor has identified this as a risk area in the Medicare program. This session will update you on these issues and other areas of interest to the palliative care and hospice industry found in the 2015 rule.
