New CPT Codes for Advance Care Planning and POLST/MOLST Discussions

CMS has established a separate payment mechanism and rate for advance care planning services provided to Medicare beneficiaries by physicians and other practitioners. CMS is also providing payment for advance care planning when it is included as an optional element of the “Annual Wellness Visit.”  New CPT codes for advance care planning and end-of-life discussions with patients and families have been adopted to enable billing for these conversations. The codes are effective for services provided on or after Jan. 1, 2016.
