This session will look at the integration and application of palliative care as an outcome driver for community-based home health and hospice programs as the value-based payment climate takes hold. Faculty will focus on palliative care as an emerging model of primary care that can help change your organization by improving practice, increasing your ability to provide a continuum of care across multiple business lines, or through partnerships. This session will include both operational and financial metrics to consider.
