To align value–based care and integration of services across the continuum, post-acute providers will face a number of key changes in 2016. The currents of change have begun. These transitions include value-based care, payment reform, unified payments and greater integration of care. Hospice and palliative care leaders must understand what is needed to position their organization to be an integral part of the community’s health care continuum without sacrificing their core values. Through shifts in models of care, greater employee engagement and managing for results, leaders will be able to creatively influence, strengthen and sustain their business models.

3. Identify key initiatives leaders use to create value and promote services using quality metrics
