As leaders, we provide clear vision, support and guidance that strengthen the delicate balance between regulatory requirements, the needs of our patients, and connecting to the hospice heart that brought our staff to do this work. Optum Palliative and Hospice Care developed a program weaving a culture of caring for patients living with dementia (whether their primary or co-morbid diagnosis), exploring ways to reignite their connection to self, and providing interventions designed to enhance their quality of life and well-being. Based on one program's experiences, participants will be able to create and implement a program specific to their own needs.

Learning Objectives:
1. Identify an outline for design and implementation
2. Explore two ways staff can be empowered in program implementation
3. Develop a strategy for stakeholders to increase participation and support long term utilization
