Should I Stop the Warfarin? The Role of Antithrombotics in End-of-Life Care

A cornerstone principle for healthcare clinicians is to “first, do no harm.” When caring for people near the end of life who have many risk factors for both major cardiac events and bleeding, how do we balance the do-no-harm principle in regard to antithrombotic therapy?  Determining whether or not to treat patients with antithrombotics is an individualized process, considering evidence-based medicine and patient-specific factors. With many new antithrombotics on the market and various indications for use, it is increasingly difficult for clinicians to keep up with the latest drug information. The aim of this presentation is to provide clinicians with the tools needed to make decisions on the appropriateness of antithrombotics for patients nearing the end of life.

2. Identify potential risks and benefits of antithrombotics using current drug information
3. Use risk tools, evidence-based medicine and patient-specific factors to make decisions about antithrombotic appropriateness in case studies
