At The Table: Hospice’s Role In a Rapidly Changing Healthcare Environment

In a rapidly evolving and increasingly uncertain healthcare environment, everyone is looking for strategies to continue to provide the best care possible. Despite the current uncertainty, some things are becoming increasingly clear. We have tremendous opportunities around improving how well we help patients navigate care transitions (both acutely and more long term). Likewise, we have much work to do around understanding, documenting, and putting into action what patients want for their own health. Hospice has been managing these challenges effectively for decades.  Despite this, our value often remains under-recognized and we are asked to help with patients very late in their journeys. This session will explore how hospices are uniquely positioned to help our colleagues provide the best care possible to patients when facing advanced medical illness.
