Sharp HealthCare is transforming the healthcare experience for patients living with advanced chronic illness by using evidence-based medicine and data driven results.  Building a collaborative partnership with key medical groups and hospitals, Sharp HospiceCare has found ways to better manage patients with chronic heart failure, dementia, COPD, advanced stage cancer and other diseases by bringing care to the place where the patient and family reside – to their home.  Transitions bridges the “four walls of traditional care” by expanding the principles of palliative and hospice care at an earlier point in the patient’s disease process.  Data strongly support the efficacy of the program through positive outcomes in both quality care and financial performance.
