Optum Palliative and Hospice Care has implemented a multi-disciplinary comprehensive palliative care program in collaboration with Monarch Healthcare in Southern CA. This program provides Physician/Nurse practitioner led multi-disciplinary (including Chaplain and Social Work as needed), ongoing palliative care to over 500 new patients per year for several IPAs where some of the members are also part of Accountable Care Organizations. This presentation will discuss the “lessons learned” and “nuts-and-bolts” experiences of this program. Topics to be covered will include: needs assessment, program design, physician and health-system engagement and buy-in, policy considerations and development, staff training and support, scheduling and clinical management processes, documentation, physician and health system communication, financial and clinical outcome assessment, and process improvement. Substantial time will be allotted for discussion.
