This presentation will describe the development and start-up of an outpatient palliative care center that operates as a licensed community clinic. The first of its kind in California. The Palliative Care Center Silicon Valley offers patients consultation related to advance directives and treatment options, as well as treatments to manage symptoms related to their advanced illness. The Center offers an interdisciplinary approach to care which includes Physicians, Nurse Practioner, Licensed Clinical Social Worker and Spiritual Counselors. Additionally, "Transitions", a comprehensive program offering case coordination and volunteer services has been integrated into the program model. Reimbursement for clinic services is available through Medicare Part B, Medi-Cal (Medicaid) and private insurance.
