Through a program of transitional primary palliative care patients can be given the support necessary for them to bridge one of the major gaps in our healthcare system – supportive care for persons with chronic and serious illness.  AseraCare “Prime” will be discussed as a model of such a program. Using the Nurse Practitioner to lead the inter-professional team, palliative and supportive care services are provided across the continuum of care independent of prognosis or care options.  Evidence-based practices are used to support care transitions, strengthen communication, and help reduce financial burdens for both patients and the healthcare system. The overall benefit to the community is efficient and collaborative care that reduces the time and cost of “crisis” care while supporting the patient and family as the center of care.
