Learning Objectives:
1. Verbalize the deficit experienced by hospice patients in long-term care settings with regard to their access of psychosocial/spiritual support, bereavement support, pain management, integrative therapies (e.g. aromatherapy, music therapy)
2. Initiate design of a hospice integration program that includes hospice and palliative education, joint attendance at patient care conferences and more emphasis on interdisciplinary teamwork
3. Evaluate the impact of integrated clinical documentation and care plans upon quality of care and the ability to comply with Federal and state regulations, including surveys

Greater than 30 % of Californians will die in a long-term care facility. When caring for patients in long-term care settings, hospice agencies nationwide commonly experience challenges related to care plans, pain assessment, service delivery, access to special services, clinical documentation and regulatory compliance. This session describes how the successful integration of hospice and long-term care eliminates these challenges while creating opportunities for each patient to experience the best quality care possible.
