Documenting Terminal Status: The LCDs and Beyond

1. Identify items to document nutritional status of terminally ill patients
2. Recognize functional indicators that can assist in documenting decline in function
3. Describe three factors that identify heart failure patients at increased risk of death
Regulatory scrutiny of hospice has never been greater, particularly for those patients with longer lengths of stay. This session will focus on the information needed to document terminal status for eligible patients. The LCDs will be used as a baseline but other tools that can be used to assist in determining prognosis will be reviewed.
