1. Describe how POLST can ensure that patients’ wishes for care near the end of life are respected
2. Access and discuss the up-to-date substantial evidence-base for use of POLST
3. Identify the cultural, legal, organizational and political challenges and possible solutions for optimal implementation of POLST
4. Use this information to initiate or support optimal POLST implementation in their community and state.
This session provides analysis of Physician Orders for Life Sustaining Treatment (POLST) and tools for POLST implementation and advocacy from two experienced leaders. Up-to date review of the outcome data and Implementation lessons from across the United States are discussed. Topics included are: a video case study and overview of POLST, POLST outcome data, variation in POLST forms, policy development, inter-professional process development, political, clinical and regulatory issues, and current innovations such electronic POLST registries and ePOLST.
