Learning Objectives:
1. Integrate the military history toolkit into the patient plan of care, utilizing computer-based documentation
2. Develop an education program for team members on diseases and illnesses, such as PTSD, resulting from military combat service
3. Develop a Veteran to Veteran volunteer program and form partnerships with area Veterans Affairs (VA) and military medical centers
We Honor Veterans - Presenters from The Hospice of Dayton, first hospice in the US to achieve Partner Level 3 in the We Honor Veterans national program, will give community hospices the background for creating a veteran-centric program to honor their Veterans at the end of life. Participants will receive information on integrating the military history toolkit into the plan of care, providing information on Veteran benefits, developing staff education on diseases and illnesses of military combat service, tracking Veteran demographics, developing the Veteran to Veteran volunteer program and transitioning Veterans across venues of care.
